Dr. Steele will discuss the evolution of Geisinger Health System, an integrated health services organization widely recognized for its innovative use of the electronic health record, and the development and implementation of innovative care models including ProvenHealth Navigator, an advanced medical home model, and ProvenCare program. As the nation's largest rural health services organization, the Geisinger system serves more than 2.6 million residents throughout central and northeastern Pennsylvania. The health system and the health plan have repeatedly garnered national accolades for integration, quality and service, and Dr. Steele was recently named a recipient of the 2014 Gail L. Warden Leadership Excellence Award from the National Center for Healthcare Leadership.

In 2013, the NH Legislature tasked the Certificate of Need (CON) Board with developing a state health plan to provide key strategies for improving the health of New Hampshire citizens. The plan will consider what the health care system of the future should look like in terms of integrating public health and clinical care, emerging care trends, resource consolidation, patient expectations and responsibilities, and more. What should the role of government and public policy be?

1. From Volume to Value: The Changing Healthcare Reimbursement Landscape
Moderated by Andrew Cohen, Kaufman Hall & Associates

It is no secret that this is a time of transition in health care and that there are increasing financial rewards for those providing high quality, cost effective care and penalties and risks for those who do not. This discussion will examine how the Affordable Care Act and the ongoing national debate on health care has begun to shift our nation's health care system in a different direction and toward a different path. Many hospitals around the country are asking, "What's next with regard to reimbursement and care delivery?" To help provide answers to this and other questions, a senior healthcare consultant from Kaufman Hall & Associates will examine and discuss how the health care system arrived at its current station, discuss the direction it is heading and guide participants through a series of finance and reimbursement scenarios, considerations and strategies for how to compete in the changing healthcare marketplace.

2. Population Health and Accountable Care - Considerations for Hospital Trustees
Moderated by Ned Helms, Director, Institute for Health Policy and Practice, University of New Hampshire College of Health and Human Services

The health care delivery system is undergoing dramatic and fundamental change as we move from the current fee-for-service payment model to one based on population health and accountable care. For hospital trustees, what does that mean and why is it important to your role as a board member of your community hospital? What questions should trustees be asking? And what should board members be doing now to ensure that they can continue to serve the patients and communities who depend on them?

Moderated by Tanya Lord, MPH, PhD, Patient Family Engagement Consultant for the American Hospital Association; Institute for Healthcare Improvement and Massachusetts Coalition for the Prevention of Medical Errors, and national speaker and workshop presenter for Patient/Family Engagement

Panelists including an experienced patient/family advisor and hospital leaders will discuss their motivation for addressing greater levels of patient engagement, the barriers that they experienced, their current initiatives as well as their strategy and vision for patient and family engagement.

Moderated by Anne Jamieson, CEO at Portsmouth Regional Hospital and a past president of the NHHA Board of Trustees

Connecting and integrating behavioral health care treatment among different health providers and other community support services is the goal of high quality, patient-centered care and a theme for the annual meeting. Panelists will describe new efforts underway to address the challenges of providing behavioral health across the care continuum in the State.

Cancellation Policy:

Full Refunds –30 Days or More Notice
To receive a full refund, requests must be made no later than August 7, 2014.

50% Refunds –Notice of Less than 30 Days
Requests for refunds within 30 days but prior to two weeks of the start of the Annual Meeting will be subject to a 50% refund. To receive a 50% refund, requests must be made between August 8 and August 23, 2014.

No Refunds – Within 2 Weeks
Requests for refunds within two weeks of the start of the Annual Meeting will not be eligible for a refund. You will not receive a refund if you cancel any time from August 24, 2014 on.

Extraordinary Circumstances:
Refunds may be granted if an attendee is unable to attend the Annual Meeting due to a family death, illness, or other extraordinary circumstance. In such a circumstance, NHHA must be contacted by phone, letter or e-mail.

Substitution Policy:
Registrants may send a substitute in their place in lieu of requesting a refund.

NHHA will refund fees as soon as possible and no later than three (3) weeks after the Annual Meeting has concluded.
